I, the undersigned, as the parent or legal guardian of the player listed below understand and acknowledge that there are certain inherent risks in Player participating in Little League Baseball, or any other youth baseball, and have voluntarily assumed those risks on Player’s behalf by granting Player permission to play such baseball. I also understand and acknowledge that protective equipment cannot prevent all injuries Player might receive while participating in such baseball. Therefore, in consideration of Player participating in a Lamar select baseball game or event, be it in tournament play or otherwise, at the Lamar Little League complex at George Park or on any of its grounds, I, on behalf of myself and Player, hereby release the organizers, volunteers, and participants of the select baseball game, be it in tournament play or otherwise; Lamar Little League, The Lamar Sports Association, any Lamar select baseball team, its directors, officers, members and volunteers; and the City of Richmond from liability for any and all injuries, regardless of the cause of such injuries or whether such injuries are anticipated or unanticipated, on the field or off, which may be sustained by Player during participation in any Lamar select baseball game or event, be it in tournament play or otherwise, at the Lamar Little League baseball complex at George Park or on any of its grounds.

I further agree that all Lamar select baseball game or event organizers, volunteers and participants; Lamar Little League, its directors, officers, members and volunteers, The Lamar Sports Association, its directors, officers, members and volunteers, all Lamar baseball teams; and the City of Richmond shall be saved and held harmless by me from and against any and all claims, damages, losses or liability of any type for any injury to my child, received or sustained from my child’s participation in a Lamar select baseball game or event, be it tournament play or otherwise, held at the Lamar Little League baseball complex at George Park or on any of its grounds, including claims and damages arising in whole or in part from the negligence and/or gross negligence of Lamar select baseball game or event organizers, volunteers and participants; Lamar Little League, its directors, officers, members and volunteers, The Lamar Sports Association, its directors, officers, members and volunteers, any Lamar baseball team; and the city of Richmond.

Copies of Birth Certificates may be required at any time during the tournament, so please have them with you. Not having the Birth Certificates when requested could result in forfeiture of all games played during the tournament.
